Meanwhile, Castiel is struggling with his own issues, feeling disconnected from his duties as an angel and questioning his purpose. He tries to reach out to Bobby, but the old man is still wary of the angel's intentions.

Sam and Dean are investigating a series of bizarre occurrences in a small town where people are experiencing strange and terrifying hallucinations. The brothers soon discover that the source of the hauntings is a former mental patient named Patch (played by Michael Trevino), who was subjected to inhumane treatments and experiments at a local asylum.

Patch, a serial killer with the ability to manipulate people's perceptions of reality, has escaped from the asylum and is using his powers to torment the townspeople. As Sam and Dean try to track down Patch, they begin to experience strange and disturbing hallucinations themselves, making it difficult for them to distinguish reality from fantasy.
